Gift aid forms - 2009-06-17

HMRC has published a new version of the Gift Aid form with a slight amendment to the declaration in relation to income tax and/or capital gains tax that must be made by the donor. A copy of the revised form is available at http://search2.hmrc.gov.uk/kbroker/hmrc/forms/viewform.jsp?formId=734
